Diagnostic anténatal, échographie obstétricale et périnatalité

Pour devenir un espace de prévention pluridiciplinaire, le diagnostic anténatal en général et l’échographie obstétricale en particulier invitent au dépassement du clivage entre spécialistes du soma et de la psyché à la maternité.

Dans sa pratique coutumière, le cadre échographique joue désormais un rôle sensible dans l’évolution du processus de parentalité. Il induit une effusion psychique parentale en amplifiant les conflits inhérents au processus de parentalité prénatal. Lors de cette rencontre, parents et professionnels sont dans une étroite intersubjectivité. La menace ou la confirmation d’une anomalie fœtale, pouvant déboucher sur une IMG, donne une tonalité potentiellement tragique à cette démarche échographique de diagnostic anténétal.

Dans ce contexte de créativité et de vulnérabilité, l’échographiste est un médiateur/traducteur. À cette place, il peut favoriser la potentialité structurante de cet examen mais, tout autant, en stigmatiser la valence désorganisatrice.

In a preventive goal, antenatal diagnostic and obstetric ultrasound require multidisciplinary cooperation between those concerned with the physical and psychological aspects.

It plays a fundamental role in the process of parenthood even in the most ordinary of cases. It’s psychological impact exists on all the people involved (intersubjectivity between parents and health professionals). The potential for tragedy occurs as soon as there is a suspected or confirmed abnormality and the possibility of interrupting the pregnancy arises. The psychological impact of the ultrasound on the parents can lead to upsetting memories, especially when past conflicts are revived during the prenatal period.

The echographer therefore acts as a mediator/translator in this meeting where there is an atmosphere of vulnerability and creativity. He can help the couple to use the raw images as organizers for their parenting rather than allow destructive effects to develop.
